*12  Observed employee during food prep touch her nose with her gloved hand and return to food prep without changing gloves or washing hands. Also observed same employee change gloves without washing hands prior to putting on a new pair of gloves. When using rubber gloves, hands must be washed prior to putting gloves on and when changing tasks. Please instruct employee on the proper hygienics and proper use of rubber gloves to prevent any cross contamination. (750.530d) Next Inspection
*20  Observed the dish washing machine sanitizer to be at 0 ppm. Please provide for dish machine sanitizer to be 50-100 ppm. DISH WASHING MACHINE CANNOT BE USED UNTIL REPAIRED AND SANITIZER CONCENTRATION IS 50-100 PPM. THIS MUST BE VERIFIED BY A MEMBER OF THE COUNTY HEALTH DEPARTMENT BEFORE USE. (750.830) Next Inspection
Observed various foods in freezers and refrigerators not properly labeled. Once out of original container, please label with item and date; all previously cooked then cooled food must be labeled with item, date and when cooling began. Please label all foods properly or they will be discarded. (750.100) Next Inspection
15  Observed the following non-food contact surfaces to be improper: Lower Level Kitchen: 1. chipped counter tops, 2. rust on wire racks, 3. chipped wood on all wood shelves. 4. warped wood on counter top in men's restroom. 1st floor kitchen: 1. broken counter top of hand sink, 2. cracked and insulation showing on inside of refrigerator door, 3. rust on lower shelves of prep table. Waitress station area: 1. rust on inside shelves of chemical cabinet under hand sink, 2. chipped wood on lower shelves of hutch, 3. broken piece off of door to two door cabinet. Bar area: 1. rust on lower shelf of table next to refrigerator, 2. rust on inside of bar coolers. Please repair or replace; or repaint and seal. All non-food contact surfaces must be smooth, easily cleanable, corrosive free, and non-absorbent. (750.690) Next Inspection

Waitress Station: 1. shelves of hutch. Bar area: 1. inside of cabinets and all shelves, 2. call racks, 3. under glassware on top of beer coolers, 4. soda gun holder, 5. top of bar counter, 6. inside of napkin/stir holder by hand sink.
